In 2008 we embarked on a mission to re-design Casenet’s Care Management Software offerings to address the fact that each customer of a Health Management organization has unique and evolving requirements. Furthermore, to maximize utilization cost containment, segmented populations within those customers must be managed uniquely and members must be uniquely engaged.

After 8 years of experience in the care management solution business, the convergence of Healthcare Reform and evolving trends, such as Medical Home and Total Population Management, necessitated a new paradigm in care management system design that exploited new technology. Going forward, we understood that the success of our clients depended on their ability to easily accommodate idiosyncrasies within segmented populations while controlling their own destiny without having to rely on their IT departments or Casenet. In 2009, we delivered on these requirements with the launch of TruCareTM.

Our TruCare Care Management Software was built on the foundation of an innovative healthcare-specific functional architecture we call Differential CareTM. Differential Care Management (DCM) finally enables health management organizations to expand and scale their operations to meet and exceed customer expectations while targeting segmented populations as well as member-specific requirements.

Not just Member Centric but Targeted Population Centric
  • Until DCM, most industry solutions, at best, have a very limited capability to segment insured life populations
  • Only Casenet’s TruCare Product offers member centric processing and targeted insured life Care Management (CM) program configurability built into the core infrastructure of the product
  • As such, TruCare can manage fully configurable CM programs for unlimited numbers of targeted populations within:

    • Health Plans
    • Employer Groups
    • Medicaid or Medicare programs
    • Customers of UM/DM/CM carve outs
    • Any other segmented population having its own risk characteristics

    • A hierarchy of 9 levels of identification of targeted populations
    • CM Programs assigned to every targeted population
    • Staff Roles and Responsibilities assigned to each targeted population
    • A Rules Engine to drive rules throughout the system specific to each targeted population
    • Correspondence Generation (letter content, letter format, logo, etc.) specific to each population
    • Assessments, Care Plans, Structured Notes, Queues all configured to CM programs assigned to populations
    • Integration Module(through use of a fully integrated ETL tool) configured to assign members to targeted populations
